Vaccination Clinic for WVUSD Families and Community Members - Ages 12 and Older

 

Hello Walnut Valley Families,

We are pleased to offer the final 2021 COVID-19 vaccination clinic opportunity for Walnut Valley families and community members.  This clinic will be held in the Diamond Bar High School theater on Saturday, December 4 from 9:00 a.m. to 12:00 p.m.  This clinic will be for anyone who is eligible for the booster, and anyone who needs to get their second dose of the vaccine only.  The first dose of the vaccine will not be given again at this location until January 8, 2022.  When we start in January, we will continue with an opportunity every third Saturday.

WHAT TO BRING:

  1. A copy of your proof of age (ex. Birth certificate, passport, AIRES cover page for students) – we will need to keep the copy
  2. Proof of insurance if applicable – we will need to keep a copy
  3. Minors must be accompanied by a parent/guardian

If you are able to provide us with a copy of your proof of age and insurance it will save valuable time for you and for the clinic.
